Cardiac Rehab

In partnership with the Cardiac Rehabilitation unit at Derby Hospitals NHS Trust, our Phase IV Cardiac Rehabilitation programme offers move on community-based lifestyle and physical activity sessions for those who have completed/nearly completed their Phase III Cardiac Rehab. The programme aims to improve lifestyle through supporting changes in diet, smoking cessation and increasing physical activity levels. It will also give people the opportunity to meet other people who are going through similar experiences.

Exclusion criteria:

  • Recent Complicated Myocardial Infarction
  • Unstable Angina
  • Uncontrolled Ventricular Dysrhythmia
  • Uncontrolled Dysrhythmia Compromising Cardiac Function
  • Third Degree Heart Block
  • Aortic Stenosis
  • Aneurysm Including Ventricular Aneurysm
  • Acute Illness
  • Thrombophlebitis
  • Thrombo-embolism
  • Acute Congestive Heart Failure
  • Significant Emotional Distress
  • Pericarditis and Myocarditis
  • Recent ECG changes suggesting other Acute Cardiac events
  • Severe Hypertension (180/110+)

Please note, you will need to submit a completed BACPR form to [email protected] alongside this referral. Patients will be contacted within five working days but cannot start the programme without this paperwork.
