Physical Activity & Health Officer (Adults - PCN South)

DCCT Grade 3: Salary - £21,735 - £25,357.50 (full time equivalent). 

Candidates will normally be offered a starting salary at the lower end of the range.

We care about our community; we care about our people.   We have an exciting vacancy within our Health department for a Physical Activity & Health Officer to join our successful team delivering targeted health intervention programmes, aimed at reducing health inequalities and promoting healthy lifestyles to people living in Derbyshire.

Amongst several Health and Wellbeing programmes within the health department, we work in partnership with the Primary Care Network (PCN). We have recently secured additional funding to deliver a 12-month physical activity and health programme around supporting frequent attenders to GP surgeries using physical activity and behavioural change technique to engage the target audience and reduce their attendance.

The role will include supporting clients to make positive and sustained lifestyle choices over a 12-month period, with a focus on motivating individuals and/or groups, increasing their physical activity level whilst providing healthy lifestyle advice and guidance.

You will be a Level 3 qualified Personal Trainer, with demonstrable experience of supporting individuals with specific health concerns on a one-to-one and group work basis to make positive healthy lifestyle changes.  With an empathetic and person-centred approach, you will possess the ability to engage people into the programme, inspiring and motivating others towards behavioural change.

This role is full time (35 hours per week) and initially fixed term for a period of 12 months.
