We have an exciting opportunity for 2 Physical Activity & Health Officers to join our Health team, working in partnership with Primary Healthcare Derby Ltd and the Primary Care Networks (PCNs) to deliver a 24 month physical activity and health programme aimed at supporting patients living with long term health conditions and supporting people to live well with pain.
Within this role you will work in partnership with social prescribers and GP managers to identify and engage the target audience, deliver lifestyle interventions around physical activity, nutritional support and health education using behavioural change models.
The ideal candidate will have experience of supporting individuals with specific health concerns on a 1:1 and group basis, together with experience of prescribing and delivering physical activity and exercise. You will be an excellent communicator, skilled in working with integrity, professionalism and discretion, you will possess and demonstrate an empathetic, person-centred approach with the ability to inspire and motivate others.

DCCT Grade 3: Salary - £22,373.73 - £26,102.69 (full time equivalent). Candidates will normally be offered a starting salary at the lower end of the range.
