NCS Pre-Programme Covid Questionnaire

Does the young person have Covid symptoms or are they currently self isolating due to a positive Covid-19 test?
Is the young person currently isolating because someone in the household is self isolating, they have come into contact with someone who tested positive for Covid-19 or the track and trace system has required them to do so?
Does the young person look after or have regular contact with someone who is vulnerable or shielding?
Is the young person at 'high' or 'moderate' risk of Covid-19 according to the current NHS definitions?

Please complete this form ahead of your first day on our NCS programme. It should be completed no more than 72 hours in advance of your start date.
