I hereby give formal consent for the young person named above to participate in the programme.  My permission is granted in full knowledge of the possible consequences regarding code of conduct breaches in relation to zero tolerance rules and behaviour.

I have been provided with knowledge of what the young person will be doing on programme, how they will be supported and understand each activity has been appropriately risk assessed and will be delivered by suitably trained instructors.

I give my consent for the young person to receive emergency medical treatment and for dietary and medical information to be shared with other venues the programme will use.

Official photographs and videos may be taken and publicised.
