By selecting an option below, you confirm or deny that you give permission for your child to be photographed/videoed for the uses outlined above, and for their voice to be heard. 

I confirm that I understand the proposed uses for the photographs/videos and that I can withdraw the photo/video from the media library at any time by contacting Derby County Community Trust or Derby County Football Club.  

I understand that there will be no payment made to me if my child’s image appears in any of the above.   

First Aid Consent

If named participant is injured I acknowledge the right of Derby County Community Trust staff to administer first aid within the parameters to which they are qualified.

Data Protection Consent

Derby County Community Trust may collect information in relation to the participant and parents/guardians to enable them to effectively administer sessions. Such information may be used in accordance with the provisions of GDPR and the Data Protection Act 2018. All data passed to funding parties will be anonymised.
